The early symptoms of hypothyroidism are very subtle and can often be confused with symptoms of other health conditions. If you have a mild case of hypothyroidism you may not even exhibit any symptoms or signs of the condition, making it almost impossible to diagnose until the condition worsens over time. As the metabolic functioning of the body slows down, various symptoms start becoming more evident and a diagnosis is possible.
A clinical trial investigating symptoms found that patients receiving l-thyroxine monotherapy, even with a normal TSH, displayed substantial impairment in psychological well-being compared with controls of similar age and sex (3). Because some hypothesized that this phenomenon came about only after adoption of l-thyroxine monotherapy, a study assessed combination therapy with l-thyroxine and l-triiodothyronine. Remarkably, the latter study showed that psychological measures improve in patients receiving combination therapy until serum TSH level is normal (6). In another study comparing l-thyroxine monotherapy versus desiccated thyroid, in which both groups had a normal TSH, many patients preferred desiccated thyroid and lost weight (60). Unfortunately, the solution to this complex problem is not as simple as reverting to combination therapy; the more than a dozen clinical trials on the subject have not shown benefit of superiority and preference for combination therapy, as previously reviewed (1, 3, 70).

Although it’s not a very common cause, sometimes newborns are born with a dysfunction of the thyroid gland, a genetic condition called congenital hypothyroidism. Some evidence shows that people are more likely to develop hypothyroidism if they have a close family member with an autoimmune disease. But according to the National Institute of Health (NIH), the likelihood of genetic hypothyroidism is very low and only about one out of every 4,000 newborns is born with a thyroid disorder.

This can lead to low T3 levels (58). In addition, elevated cortisol will cause thyroid hormone receptor insensitivity meaning that even if T3 levels are high enough, they may not be able to bind normally to receptor sites. And when this happens it doesn’t get into the cells. Cortisol will also increase the production of reverse T3 (rT3), which is inactive (11).
The thyroid gland is located at the base of the neck just below the Adam's apple. The thyroid produces two main hormones called T3 and T4 which are transported in the blood to all parts of the body. These hormones control the rate of many activities in your body including how fast calories are burned and how fast or slow a person’s heart rate is. Combined, these activities are often referred to as the metabolism. When thyroid disease occurs and the thyroid gland is compromised it may produce too few hormones and this can result in the metabolism slowing down. The scale was calibrated so that a normal BMR reference range would be around 0%, whereas athyreotic individuals could have a BMR of about −40% (21). Because of lack of specificity (for example, low BMR in malnutrition), BMR was used in conjunction with the overall clinical impression; a low BMR in the setting of high clinical suspicion would secure a diagnosis and justify treatment (21, 22).
Refined Flour Products — Any food made with refined carbohydrates, like enriched wheat flour, for example, negatively impacts hormone levels and can contribute to weight gain. Refined flour products include bread, cereals, pastas and all baked goods. If possible, remove most grains from your diet altogether. Or at least try to greatly limit the amount of products you eat that are made with any flour by choosing 100 percent whole, ancient grains instead (like quinoa, buckwheat, etc.).
Thank you for the information. I’ve been on the meds four years and I keep on getting fat and sluggish plus all the other symptoms. I’m exhausted. I also want to share that the medication and a good diet don’t cure this disease in everybody. A reliable herbalist told me it’s possible to replace the medication for a natural iodine source; to slowly lower the synthetic medication as I start ingesting algae supplements. It’s very dangerous to stop talking the synthetic med all at once. So the switch has to be very slow into the algae. I’m gonna order the algae and start giving it a chance.
Your mood is especially susceptible to changes in hormone levels, so some people with hypothyroidism deal with depression, anxiety, trouble getting good sleep and low immunity. The thyroid gland helps regulate chemical messengers called neurotransmitters, which control your emotions and nerve signaling. The most common thyroid condition is hypothyroidism, or underactive thyroid. In the United States, hypothyroidism usually is caused by an autoimmune response known as Hashimoto’s disease or autoimmune thyroiditis. As with all autoimmune diseases, the body mistakenly identifies its own tissues as an invader and attacks them until the organ is destroyed. This chronic attack eventually prevents the thyroid from releasing adequate levels of the hormones T3 and T4, which are necessary to keep the body functioning properly. The lack of these hormones can slow down metabolism and cause weight gain, fatigue, dry skin and hair, and difficulty concentrating (see table).2 Hashimoto’s affects approximately 5% of the US population, is seven times more prevalent in women than men, and generally occurs during middle age.3
The diagnosis of “subclinical” hypothyroidism that I discussed last week depends on having a TSH level higher than 5 m IU/ml and lower than 10 m IU/ml. As I mentioned above, new guidelines suggest anything over 3 is abnormal. While an improvement, practitioners following these guidelines may still miss many people who have normal test results and a malfunctioning thyroid system.
There is little mention of patients who did not respond symptomatically to treatment despite having normalization of their other measured variables, such as BMR or serum PBI, in the early clinical trials in the 1940s through 1960s. After the 1970s (38, 52), a new category of hypothyroid patient was recognized: the patient who received thyroid hormone replacement therapy, had normal serum TSH, and exhibited residual symptoms of hypothyroidism. Initially, such symptoms were largely dismissed as unrelated to the thyroid condition (62). Indeed, hypothyroidism is prevalent, and symptoms overlap with those of other common conditions, including menopause, depression, and chronic fatigue syndrome. Likewise, thyroid hormone had been administered for nonthyroid disorders, including obesity and psychiatric disease, for decades. Thus, it was difficult to assess whether patients with residual symptoms had been misdiagnosed. Residual symptoms were even attributed to nonadherence (63).
goitrogens are foods that can interfere with thyroid function. Goitrogens include broccoli, Brussels sprouts, cabbage, cauliflower, kale, kohlrabi, rutabaga, turnips, millet, spinach, strawberries, peaches, watercress, peanuts, radishes, and soybeans. Does it mean that you can never eat these foods? No, because cooking inactivates goitrogenic compounds and eating radishes and watercress in moderation isn’t going to be a deal-breaker.
Large predator fish—tuna, swordfish, shark, kingfish, mackerel—often have more mercury than smaller fish, as they’ve lived longer and had more time to accumulate harmful chemicals. Don’t eat more than two to three servings of these fish a week, Blum says. Also, farmed fish like salmon can have higher levels of mercury because they’re often fed the chum of other fish. All fish have a little mercury, so don’t freak out about it. Just don’t order in sushi every weeknight.
Iodine: Iodine is critical for thyroid hormone production in the body. A true iodine deficiency will cause hypothyroidism (43). In western culture we often see subclinical iodine deficiencies which contribute to hypothyroidism (44). I typically don’t recommend high doses of iodine as it could be problematic with individuals with Hashimoto’s – especially with TPO anti-bodies.
Alcohol consumption can wreak havoc on both thyroid hormone levels in the body and the ability of the thyroid to produce hormone. Alcohol appears to have a toxic effect on the thyroid gland and suppresses the ability of the body to use thyroid hormone. Ideally, people with hypothyroidism should cut out alcohol completely or drink in careful moderation.
