It’s imperative dietitians have a good understanding of the metabolic changes associated with thyroid disease so they can set realistic goals and expectations for clients. Most people with hypothyroidism tend to experience abnormal weight gain and difficulty losing weight until hormone levels stabilize. Moreover, it’s common for patients with Graves’ disease to experience periods of high and low thyroid hormone levels, so it may take several months to achieve a balance. During this time, it’s essential clients focus on healthful behaviors such as eating nutritious foods, exercising regularly, managing stress, and sleeping adequately rather than focus on the numbers on the scale.

Goitrogens are naturally occurring substances in certain foods that interfere with the production of thyroid hormones (the hormones that people with hypothyroidism lack). They include some of the most commonly consumed foods of the health-conscious community: broccoli, cauliflower, kale, spinach, radishes, soybeans, peanuts, pine nuts, peaches and millet. On the flip side, there are certain foods that people with underactive thyroids should minimize or avoid altogether, like cruciferous vegetables, particular raw Brassica vegetables like broccoli, cauliflower, cabbage, kale, soy, and Brussels sprouts. While these are healthy foods for most people, they contain a compound called goitrogens, which might impact thyroid function by impairing thyroid peroxidase. Gluten, conventional dairy, refined sugar and refined flour, caffeine and alcohol (which stress your adrenals) are also contraindicated for hypothyroid patients.

Large predator fish—tuna, swordfish, shark, kingfish, mackerel—often have more mercury than smaller fish, as they’ve lived longer and had more time to accumulate harmful chemicals. Don’t eat more than two to three servings of these fish a week, Blum says. Also, farmed fish like salmon can have higher levels of mercury because they’re often fed the chum of other fish. All fish have a little mercury, so don’t freak out about it. Just don’t order in sushi every weeknight.

Hyperthyroidism, or overactive thyroid gland, is another common thyroid condition. However, the condition may develop so slowly that many patients do not realize that their body has changed, so it is critically important to perform diagnostic laboratory tests to confirm the diagnosis and to determine the cause of hypothyroidism. A primary care physician may make the diagnosis of hypothyroidism, but assistance is often needed from an endocrinologist, a physician who is a specialist in thyroid diseases.


However, iodine intake has dropped during the past few decades. Americans get approximately 70% of their salt intake from processed foods that, in the United States and Canada, generally don’t contain iodine. A 2012 Centers for Disease Control and Prevention report indicates that, on average, Americans are getting adequate amounts of iodine, with the potential exception of women of childbearing age.10
People with celiac disease who can’t tolerate the gluten found in many baked goods, pasta and cereals often have Hashimoto’s thyroiditis, and vice versa. Hashimoto’s disease is an autoimmune condition in which your immune system attacks your thyroid. Once rare, Hashimoto’s is now the most common autoimmune disease, according to the May 2017 study in the journal Endocrine Connections.
If for some reason the pituitary gland or the hypothalamus are unable to signal the thyroid and instruct it to produce thyroid hormones, it may cause decreased T4 and T3 blood levels, even if the thyroid gland itself is normal. If pituitary disease causes this defect, the condition is called "secondary hypothyroidism." If the defect is due to hypothalamic disease, it is called "tertiary hypothyroidism."
“Infants fed soy formula are at higher risk for hypothyroidism and for later development of autoimmune thyroid diseases. In humans, goiter has been seen in infants fed soy formula; this is usually reversed by changing to cow milk or iodine-supplemented diets . Seaweed has a high concentration of iodine, an essential nutrient for thyroid function. "Iodine is the precursor for the production of thyroid hormone," Dr. Dodell explains. Seaweed, packaged as nori, wakame, and dulse, can be used in sushi, soups, and salads. Another plus: Seaweed offers nutritional benefits of fiber, calcium, and vitamins A, B, C, E, and K.
If you have thyroid issues, then raw cruciferous vegetables may not be the best choice. You might want to skip the kale smoothies and salads, and eat your greens cooked instead. The reason is that the cruciferous vegetables contain goitrogens that may disrupt the thyroid if consumed in large quantities. Other cruciferous veggies include cabbage, Brussels sprouts, broccoli and cauliflower.

Certainly, many of the foods listed above are an important part of a healthy diet. Try to eat a varied diet so that you avoid eating large amounts of goitrogenic foods on any one day.
